Guidelines

The Sunset Wish Foundation accepts referrals for potential wish recipients from family, friends, medical professionals (such as a social worker, doctor, nurse, or other specialists), or the adults themselves. If you are not eligible to refer an adult, but know of an adult who may be eligible for a wish, please have the adult’s family or friends contact us.

To request a wish, potential recipients must complete a Wish Request application.  The easiest way to request a wish is to click the link above which will automatically take you to our online Wish Request application.  All wish applicants are encouraged to make realistic choices that fulfill a wish from the heart.

We CANNOT grant the following types of wishes:

· Requests for a terminally ill individual who is not in the last year of life

· Requests for terminally ill individuals under the age of 18

· Requests for cash, property, automobiles, computers, or home improvements

· Requests for travel outside of the United States

· Requests for cruises

· Requests to provide legal assistance, medical treatments, or equipment

· Requests for terminally ill individuals who are not residents of the United States
